Technology rarely fails on its own — in the three stalled transformation projects we reviewed for this piece, the software worked as specified in every case.
What broke down was adoption: staff reverting to familiar manual processes under deadline pressure, middle management left out of the rollout plan and quietly undermining it, and training scheduled too close to go-live to actually stick.
The common fix across all three, once we got involved, wasn't a technical patch — it was slowing the rollout down and rebuilding trust with the people expected to use the system daily.
